Job Title: Community Connector
 
Responsible to: Healthbox CIC management team
 
Salary: £24,000 per annum

Main Purpose of Role
 
Community Connectors are members of the community who know what services and activities are available in their local area and can signpost friends, family, neighbours and colleagues to these community activities. Their main role is to help build a bridge for local people to other services, building local community capacity and supporting residents.
 
Social prescribing is a way of linking patients from their GP practice to access the activities and support in their local community. The Community Connector will help the social prescribers, their referred patients and the wider community make the most of the assets in their local area. In effect, they help map and navigate the range of support available for people in their neighbourhoods.
 
As part of Connected Communities in Macclesfield, we are looking to employ a Community Connector to work alongside the Connected Communities Team to encourage uptake of local services.
